As a member of the healthcare team here at Riverview Health, the Documentation Specialist at Westfield Medical demonstrates active patient advocacy to ensure all rights are identified and met for every patient. The Documentations Specialist encompasses all duties relating to providing exemplary housekeeping services at Riverview Health. Outstanding customer service skills and understanding of confidentiality and productivity are required by the Documentation Specialist.


  • May perform all pre-visit planning to review previous notes and obtain any pre-visit laboratory and diagnostic test results, review for gaps in care and preventive health measures, and help to assess and support patient’s self-management of chronic health conditions.
  • Interacts with patients and provides clinical support to obtain and record past medical family and social histories, including updates to past histories and review of systems related to reason for visit. Prepares visit summary for patients. May assist patients with enrolling in the patient portal.
  • May prepare patients for physician/provider examination and administers injections and immunizations. Performs laboratory testing, cleansing wounds/lesions sites, EKG's and other medical office testing and procedures. Measure and record at least three vital signs per patient visit.
  • Responsible for E.H.R. support to enter and pend orders for provider signature, document laboratory and radiology findings, enter CPT and ICD 10 codes as verbalized by the physician, locate past medical history, previous notes, recent studies and other required information at the providers request.
  • May also perform post-visit follow-up to that all future test and medication orders, referrals, and patient education are complete. May also ensure that patient’s requiring any follow-up visits are scheduled according to the physician’s care plan. Accountable to arrange for diagnostic testing procedures, specialist appointments and hospital admission.
  • Facilitate improvement in efficiency and production, such as, patient flow and the ordering of tests and patient medication, with a team concept focus and exemplifies excellence in service delivery. Works with both front office and clinical areas as assigned.
  • May Perform telephone triage asking details of illness, answering questions, ensuring information is available, and reviewing phone calls with the nurse practitioner or physician as appropriate.
  • Maintain waiting room, examination rooms and offices.
  • Train other Medical Assistants. Complete all educational requirements.
  • Notify management of changes in workload. Perform other duties as assigned.

Job Requirements


  • Level of Education
    • Minimum: High School Diploma or equivalent and Completion of qualified Medical Assistant course.   CMA or RMA certification.
  • Level of Experience
    • Preferred: 1 or more years of Medical Office experience, previous patient care experience in the specialty applied for.
